My Health Passport uses Widgit symbols and contains three sections:
- Things you must know about me (Red)
- Things important to me (Amber)
- My likes and dislikes (Green)

My Health Passport has been designed to support and facilitate effective communication and shared understanding between the Child and Young Person, (CYP) family and professionals who are caring and providing support.
This tool will be part of the child and young person's journey for some, this will include transiting into adult services, for others this may be in place due to a Learning Disability or Complex Health Need. The My Health Passport will be used by the Multi-disciplinary Team to allow collaborative working.
The Health Passport is a vital element based on the provision of effective communication that allows and encourages others to understand the needs of the CYP especially during acute or traumatic episodes. The My Health Passport will highlight reasonable adjustments to enable a positive outcome.
My Health Passport is a document which will empower the Child and Young Person and their families to share with health professionals and other professionals to promote the communication and share the information about their wishes, values and daily routines. The communication tool which will enable reasonable adjustments and support the child, young person and family and avoid unnecessary delay. This tool will be part of the child and young person's journey for some, this will include transiting into adult services, for others this may be in place due to a learning disability or a complex Health needs. This Tool can be shared with schools, Therapies and Disability teams to work in partnership.
